You can order your repeat prescriptions by ringing 01709 533050 Monday -Friday between 10.00am - 4.00pm

To provide geater flexibility and convenience, particularly for workers who struggle to attend during the day, the surgery is open on Monday evening 6.30pm - 8.00pm. This is strictly on an appointment only basis.

New Patient Registration

We currently have an open list and welcome registrations from people living in or  moving to the practice area  

If you move out of the area we regret you will need to register with another practice.  You are registered with the Practice rather than an individual GP and may see any of the doctors.  You are able to express a preference of  GP and whilst the practice will try to comply it may not be always possible in which case we will tell you why.  

To register please call in and collect a pack from reception.  You will need to bring proof of your identification (photographic, i.e passport) and address before full  registration can take place.

The practice uses the electoral role when checking addresses where discrepancies  have occurred

You will be given a form to complete to bring with you for you New patient Medical

It is important that you keep us informed of any changes to your circumstances i.e. address, telephone number inoder for us to be able to contact you should the need arise.


Change of Address

Please let us know as soon as possible if your address changes.  Call in at the surgery and complete the form to enable us to amend you details.

Please make sure that we have an up to date contact phone number. If you have given us a mobile number, please let us know if you change it, there are occasions when we need to contact you, and this can cause delays if a mobile phone number is out of date. We are happy to contact you via e-mail, so please feel free to give a current e-mail address.
