Nurse Associate

A Nurse Associate is a member of the nursing team that helps to bridge the gap between health care assistants and registered nurses. They work with people of all ages, in a variety of settings in health and social care. Their role contributes to the core work of nursing, freeing up registered nurses to focus on more complex clinical care. Along with carrying out routine Healthcare Assistant procedures, Lucie leads on the new NHS Health Checks for patients between 40-74 years.

Pharmacists

Clinical pharmacists work in primary care as part of a multidisciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas. Many will also be prescribers. They work with and alongside the general practice team, taking responsibility for patients with chronic diseases and undertaking clinical medication reviews to proactively manage people with complex prescriptions, especially for the elderly, people in care homes and those with multiple medical conditions.

Social Prescriber

Social Prescribing Link Workers give people time and focus on what matters to the person as identified in their care and support plan. They connect people to community groups and agencies for practical and emotional support and offer a holistic approach to health and wellbeing, hence the name ‘social prescribing’. Link workers typically work with people over 6-12 contacts (including phone calls and face to face meetings) over a three-month period with a typical caseload of up to 250 people, depending on the complexity of people’s needs.

The role of a GP Assistant is very new to Primary Care. It has been created to deliver clinical and non-clinical duties supporting GP’s in their day-to-day management of patients, specifically aimed at reducing the administrative burden

Paramedic

The role of the Paramedic in General Practice is to carry out clinical assessments and treatment on all patients over the age of 1. They assist with the urgent same-day appointment request workloads, but also have pre-bookable appointments for minor conditions.

This is a specialist nurse role who work with patients and families affected by dementia. They provide consultancy and support to health and social care colleagues to improve the delivery of evidenced based dementia care.
